Complete your forms

New patients receive portal access when the appointment is booked. Completing and signing your paperwork in advance means no clipboard on arrival.

Insurance & billing

We take most plans.

Our offices participate with most commercial health insurance plans and Medicare. Covered procedures vary by insurer, and our staff will help you determine eligibility before your visit — just call with your card to hand.

What we accept

Most commercial plans and Medicare, including Aetna, Cigna, Horizon BCBS, Humana, UnitedHealthcare and more.

We do not participate with any Medicaid plans.

What you’re responsible for

Deductibles, co-payments and any remaining balance after your insurer has processed the claim.

Bills can be paid securely online, free of charge.

Appointment policies

Cancellations & no-shows.

We work hard to keep waiting times minimal and your experience in our offices pleasant. Please arrive fifteen minutes prior to your scheduled appointment time, and call the office at least 24 hours in advance for any cancellations.

Medical appointments

$50

If you do not cancel at least 24 hours before your scheduled time, the appointment is considered a no-show and a $50 fee is charged.

Cosmetic appointments

48 hours

A non-refundable deposit is taken when the appointment is made. If you do not cancel at least 48 hours in advance, the deposit is not refunded.

The deposit is applied to the cost of your treatment.

Yes. We treat patients of all ages, from infants through to older adults.

Dermatologists treat a wide range of conditions affecting the skin, hair and nails — from acne and eczema to skin cancer.

Only a dermatologist can distinguish a benign skin growth from a cancerous one. If you have a suspicious growth, your dermatologist may need to perform a skin biopsy.

Excessive exposure to UV rays from the sun is the biggest risk factor for developing skin cancer. The best prevention is to limit sun exposure and wear sun protection.

Unless your insurance plan requires a referral from your primary care physician, you can simply call us or book online. To find out whether you need one, call the number on the back of your insurance card.
